I agree to hold Fitness Resources, its presenters, and sponsors harmless from any and all responsibility arising from this event, including, but not limited to, muscle pulls, tears, strains, illness, broken bones and missing, lost or stolen property.
I understand the risk of participating in these practical application workshops and declare, by checking the box below, that I am in sound physical condition. I also agree to all registration conditions listed below. By checking the box below and clicking submit I agree to and understand all terms. REGISTRATIONS WILL NOT BE PROCESSED WITHOUT SUBMITTING THIS FORM.
REFUND POLICY
- Requests for refund of registration fees must be in writing and postmarked or faxed on or before 14 days prior to the seminar. All requests for refunds will be assessed a $35.00 administrative fee. No transfers allowed.
- Absolutely no refunds (NO EXCEPTIONS) if requested 13 days or less prior to the seminar or after the seminar.
Presenters, classes, dates and times are subject to change without notice. Fitness Resources is not responsible for acts of God, nature or terrorism, which may affect venues or schedules.
